Sinnott worked primarily as an inker and is best known for his long tenure on Marvel Comics' Fantastic Four, from 1965 to 1981 (with a brief return in the late 1980s), initially over pencils by the legendary Jack Kirby. Years earlier, he had inked Kirby's The Fantastic Four #5 (July 1962), the issue that introduced Dr. Doom, and a science fiction monster story in Strange Tales #94 (March 1962).

During his more than fifty years as a freelancer and later a Marvel employee, working from home, Sinnott inked virtually all the major titles, including notable series like The Avengers, The Defenders, and The Mighty Thor. Marvel impresario Stan Lee called Sinnott the company's most sought-after inker in the mid-2000s, jokingly saying, "Artists would threaten me if I didn't make sure Joe, and only Joe, inked their pages.
